LIVMA 2025 - 2026 Award Nomination Form

LIVMA is currently accepting nominations from active members and supporters for our 2026 Member Celebration.

Awards will be presented at LIVMA's next Membership Celebration and Scholarship Dinner in June 2026.

In addition to thanking those members who have participated in LIVMA's programs throughout the year like CE presentations, Career Exploration, and more LIVMA is including honorees who go above and beyond their "normal job" to assist, promote, or influence the welfare of animals. LIVMA is asking all active members and supporters to consider nominating a veterinarian, technician, and community member who deserves our recognition.

Please complete the form below to nominate either a Long Island veterinarian, veterinary technician, or member of the community who you feel deserves to be honored for their contributions to animal welfare and veterinary medicine.
